Telehealth Nurse Practitioner – Delaware / Virtual Care Management Nurse Practitioner
Deliver high-impact virtual care for medically complex Medicaid and Dual Eligible patients while helping expand an innovative, physician-led care model across Delaware. This position offers the opportunity to improve patient outcomes through proactive care management, interdisciplinary collaboration, and telemedicine.
Why This Assignment
- Full-time (40 hrs/week)
- Schedule: Flexible schedule with participation in a shared 24/7 clinical triage rotation
- Provide virtual care management for medically complex Medicaid and Dual Eligible (D-SNP) patients
- Coordinate care transitions, including hospital-to-home follow-up and advance care planning
- Collaborate with physicians, nurses, social workers, therapists, caregivers, and community partners
- Support HCC coding, risk adjustment, and STAR/HEDIS quality initiatives
- Help develop care pathways and best practices as the Delaware program grows
- Deliver care through telehealth and remote patient monitoring
About You
- Be able to do the job as described.
- MSN, Nurse Practitioner
- Active Delaware Nurse Practitioner license
- FNP, NP, or Gerontological Nurse Practitioner certification
- At least one year of telemedicine or care management experience
- Managed Care experience preferred
- Experience with HCC coding, risk adjustment, and STAR/HEDIS quality measures preferred
