What You’ll Be Doing
- Deliver virtual clinical care and ongoing management for Medicaid and Dual Eligible members
- Lead, mentor, and develop interdisciplinary clinical team members
- Establish and scale clinical workflows, standards, and operational processes
- Perform patient assessments, medication management, care planning, and follow-up via telehealth
- Coordinate care with providers, specialists, health plans, and community resources
- Lead advance care planning discussions and support care transitions
- Ensure compliant documentation, HCC coding, risk adjustment, STAR, and HEDIS initiatives
About You
- Be able to do the job as described.
- Active Ohio NP license
- Will consider: FNP, PMHNP, AGNP, or GNP certification
- Master’s degree or higher required
- 1+ years of experience in Telemedicine Care, Care Management, or Allscripts Care Management
- Experience supporting medically complex populations in a value-based care environment
- Knowledge of HCC coding, risk adjustment, STAR, and HEDIS quality programs
