Lead Nurse Practitioner Telehealth / Telehealth Nurse Practitioner Leader

What You’ll Be Doing

• Deliver virtual clinical care and ongoing management for Medicaid and Dual Eligible members
• Lead, mentor, and develop interdisciplinary clinical team members
• Establish and scale clinical workflows, standards, and operational processes
• Perform patient assessments, medication management, care planning, and follow-up via telehealth
• Coordinate care with providers, specialists, health plans, and community resources
• Lead advance care planning discussions and support care transitions
• Ensure compliant documentation, HCC coding, risk adjustment, STAR, and HEDIS initiatives

About You

• Be able to do the job as described.
• Active Ohio NP license
• Will consider: FNP, PMHNP, AGNP, or GNP certification
• Master’s degree or higher required
• 1+ years of experience in Telemedicine Care, Care Management, or Allscripts Care Management
• Experience supporting medically complex populations in a value-based care environment
• Knowledge of HCC coding, risk adjustment, STAR, and HEDIS quality programs
